In order to obtain the best possible results from my orthodontic treatment, I understand that my cooperative efforts are just as important as the efforts of Dr. Dormanen and his team. The doctor provides the game plan, but I am responsible to follow all instructions so that I may have beautiful, healthy teeth that last a lifetime. My part is to take responsibility for the following:
- Clean teeth and gums. I will brush my teeth and gums properly each day, especially after eating meals or snacks and at bedtime. I will floss and use my fluoride gel. I will visit my dentist every six months (or otherwise, as directed) for a professional cleaning and examination.
- Wearing Appliances. If needed, I will wear elastics and other removable appliances faithfully as directed by my doctor and team. After my braces are removed, I will wear my retainers as instructed.
- Care of Appliances. I will not eat hard, sticky, chewy foods or bite on pens, pencils and fingernails. I understand that an excess of broken appliances requires longer appointments during school hours.
- Appointments. I will try to keep all my appointments and arrive on time. I will call as soon as possible if I must change my appointment and will always call ahead of time if I have something broken or loose. I realize that my longer appointments will be during school hours. I understand that rescheduling may take a few weeks and could extend treatment time if I change appointments often.
I (we) have read, understand, and agree to cooperate by following all of the above instructions. Should I (we) have difficulty with any of these, I (we) will discuss them promptly and honestly with Dr. Dormanen and his team.
